BL46 Diaphragm Gate

BL46 (Geguan, “Diaphragm Gate”) is an acupuncture point on the Bladder Meridian. It is located on the back, 3 cun lateral to the posterior midline, at the level of the lower border of the spinous process of the 9th thoracic vertebra (T9). This point is located 1.5 cun lateral to BL18. It regulates qi, harmonizes the diaphragm, alleviates pain.

Needling

Oblique insertion 0.5-0.7 cun. Caution: pneumothorax.

⚠ Pneumothorax risk: oblique/shallow only, never deep perpendicular over the chest wall.

Geguan, or 'Diaphragm Gate,' is a significant point for harmonizing the diaphragm and regulating qi flow in the chest and upper abdomen. It is particularly effective for conditions involving stagnation of qi in the diaphragm, such as hiccup, belching, and a sensation of fullness or pain beneath the ribs. It can be used to alleviate pain in the hypochondriac region and is sometimes combined with PC6 (Neiguan) for nausea and vomiting.
