This Practitioner Listing Agreement (the “Agreement”) is a contract between you (“you”, the “Practitioner”) and Dr. Kerem AL, sole proprietorship (şahıs işletmesi), operating qiboo.ai, İzmir, Türkiye (“qiboo”, “we”, “us”). It applies to your application for a directory listing and to any listing we publish on that basis (a “Listing”). By ticking the acceptance box you accept this Agreement, which forms part of the qiboo.ai Terms of Service. How we process your personal data is explained in our Privacy Policy, including the KVKK notice; the Privacy Policy is not a contract and you are not asked to accept it. If you do not agree, please do not apply.
You apply for a Listing through the listing form. We review each application and decide whether to publish it. We may edit the information for format, length and clarity, and we may refuse an application. qiboo.ai is the publisher of the Listing page.
You may apply if you are at least 18 years old and you are either (a) a practitioner who is lawfully permitted to practise where you work and who holds the qualifications you state, or (b) a person authorised to apply on behalf of the practitioner, clinic or institution the Listing describes.
When you apply, and for as long as your Listing is published, you confirm that:
Do not include in your application false or exaggerated credentials, titles or affiliations, claims that you cure a condition or guarantee results, other people's personal data without their consent, information that identifies a patient, or content that infringes anyone else's rights or breaks the law, including rules on advertising health services.
We publish the information you choose to include in your listing in order to perform this Agreement with you (KVKK Art. 5(2)(c); GDPR Art. 6(1)(b)). Your phone number and e-mail address are shown only if you tick the relevant box. You can ask us at any time to correct or remove your listing; where the listing form lets you edit or unpublish your listing yourself, you can also do it there. Details are in section 15 of our Privacy Policy.
You give us a non-exclusive, royalty-free licence to edit the information you submit for format, length and clarity, and to publish and display it on qiboo.ai, including in the directory search, for as long as the Listing is published. The licence ends when the Listing is removed; copies in our backups are deleted when those backups expire, within about 3 months.
A Listing is not an endorsement, recommendation, referral, ranking or certification of you, your qualifications or your services. Our review before publication does not confirm clinical competence. qiboo is not a party to any relationship, appointment, treatment or payment between you and a patient or other user.
You remain responsible for the information you submit, for your professional conduct and for your relationships with your patients, including clinical decisions, informed consent, your patients' data and compliance with the rules that govern your practice.
You can ask us at any time to correct or remove your Listing by writing to [email protected]. We may edit, suspend or remove a Listing if we reasonably consider it inaccurate, unlawful or in breach of this Agreement, or if it is the subject of a credible complaint; where practicable, we will tell you first. We remove a Listing at the verified request of the person it concerns, or when a court or competent authority requires it.
Sections 10 to 12 of the Terms of Service (service provided “as is”, liability, and indemnity for professional use) also apply to your application and your Listing.
This Agreement is governed by the law of the Republic of Türkiye, in line with section 15 of the Terms of Service. If you act as a consumer, you keep the protection of the mandatory consumer-protection rules of the country where you habitually reside, and you can bring proceedings in its courts. Otherwise, the courts and enforcement offices of İzmir, Türkiye, have jurisdiction, as far as the law allows. Before starting proceedings, please contact us so that we can try to resolve the issue in good faith.
We may update this Agreement. Each version has an identifier, and we record which version you accepted and when. For material changes we will tell you in advance and ask you to accept the new version; if you do not accept it, you can ask us to remove your Listing.
